The pilot survived.<\/p>\n<\/li>\n On January 16, 1978, the MiG-21 U with 39 PLM crashed at the airport in Mierz\u0119cice. Pilot (trained) lieutenant Tomasz Smaga was killed. Pilot instructor from the other cabin catapulted safely. During a school flight in NZWA near Siewierz, an engine failure occurred. During the catapulting there was no separation of the pilot from the chair and the pilot fell to the ground with the chair, dying on the spot.
